会員証の再発行をしたい

会員証の再発行はマイページより申請をお願いいたします。
会員証の再発行には、1,000円(税抜/送料込)の手数料を頂戴いたします。
一度支払い選択を確定されますと、会員証再発行の取消はいかなる理由でも対応できかねますので、予めご注意くださいませ。
会員証発送まで1〜2ヶ月ほどお時間をいただいております。

<限定版会員証をお持ちの方へ>
再発行の際は通常版でのお届けとなります。あらかじめご了承くださいませ。

■The correct email address was not registered
